In one of the final chapters of the book is an essay written by Dr Dean Kow and Dr Colin O'Toole, titled 'Considerations in determining the timing of interventions for anterior open bite repair'. In brief, after a review of many, many previous studies the authors conclude that 'there should be no preoperative correction of an anterior open bite beyond a static 5mm or in the absence of associated maxillo-facial deformity, obesity or severe malocclusion'. Other than the guidelines noted above regarding the timing of bimaxillary surgery and osseo-integration, there is generally no evidence-based guidelines that would lead us towards a specific time for open bite repair. Reasonably, speech and diet should be restored prior to referral and intervention.
Michael Brostoff, Professor of Operative Dentistry, University of Zurich, Switzerland, wrote in an editorial published in the same edition of the Journal - '"... A major effort has been made to provide a comprehensive and updated reference book, that can be regarded as an anchor of the postgraduate program of oral and maxillofacial surgery. One does not expect a textbook that is a source of reference but this one makes a great difference. The content is imbedded in the large amount of information sometimes lost in the world of misinformation that is often cited as a reference book. "..."'
"...Periodicals with a particular interest in TMJ have, in the past, failed to meet the need of clinicians for concise, up-to-date information. Yet it is the work of seasoned researchers who challenge conventional opinions, producing new insights and cutting-edge research, that is the bedrock of TMJ studies."
"... This book, which can be regarded as an anchor of the postgraduate program of oral and maxillofacial surgery constitutes, as in the first edition, one of the reference books that every oral and maxillofacial surgeon has to hand'.
